Create questions and answer choices
Choose the right text, checkbox, list, link, or picture field for each form question.
Choose a response field
Use the field that makes the intended answer obvious and requires the least effort from the participant.
Configure text responses
For Text Input and Text Area, use Validation Pattern to allow letters only, numbers only, or letters and numbers. Use Min Length and Max Length to control response length, and make the limits clear in the help text and validation error.
Configure answer choices
Write choices that are distinct and do not overlap. Avoid combining two answers into one option. Preview long choices on mobile and check how the selected value appears in participant records and exports.
For Picture Choice, add a meaningful label to every image. Choose the square, tall, taller, short, or natural aspect ratio that best fits the source images. Randomizing the order can reduce position bias, while allowing multiple selections changes the question from one answer to many.
Test the question
Test every option, a blank optional response, and the required-field error. If the field awards bonus entries, test both a completed and skipped response. Use Field ID when integrations or URL prefilling need a stable field name.
