Collect dates, ratings, and rankings
Configure date pickers, star ratings, opinion scales, and draggable rankings.
Collect a date or date range
Use Date Picker for appointments, event dates, travel dates, deadlines, or another date that is not the participant's birthday.
- Turn on Enable Date Range when the participant should select a start and end date.
- Turn on Enable Time Picker when a time should be collected with the date.
- Choose the displayed date format and customize the label, placeholder, help text, required state, and error message.
- State the relevant time zone in the question or help text when the meaning of a time depends on it.
Use Birthday instead when collecting a birth date or applying minimum-age validation to a stored participant field. Use Age Gate when age should be checked before the participant reaches the main form.
Collect a star rating
Use Star Rating for a quick score such as satisfaction or quality. Set the maximum rating from 3β10 and choose stars, hearts, thumbs, or checks as the rating icon.
Explain what the lowest and highest values mean. The icon changes presentation, not the meaning of the stored rating.
Collect an opinion scale
Use Opinion Scale for agreement, likelihood, effort, or sentiment on a numbered scale. Set the minimum and maximum values from 1β10. You can display custom labels at the low and high ends, such as βNot likelyβ and βVery likely.β
Make sure the minimum is below the maximum and define both ends of the scale in the field label or displayed endpoint labels.
Ask participants to rank options
Use Ranking when participants should put every option in order. Enter the options as a comma-separated list and customize the labels above the available and ranked columns. Participants drag items into their preferred order.
Keep the list short enough to rank comfortably on a phone. Before publishing, confirm that every option can be moved, the required error is clear, and the resulting order is understandable in participant data.
