Create a winner claim form
Prepare claim fields, legal-document settings, notifications, and translations.
Prepare and review the form before notifying winners
Open Winners and select Prepare Claim Form, Review Claim Form, or Edit Claim Form, depending on its current state. Managing the form requires winner-management permission. The claim form uses its own Form, Design, and Settings controls; changing it does not change the promotion's entry form.
Collect only the name, contact, address, eligibility, acknowledgement, or upload information needed for prize fulfillment. The form includes a required Official Rules agreement. A standalone Signature field is not available in this builder; signatures belong to the separate legal-document package when required. Review labels, required fields, validation, and the submitted message. Payment, CAPTCHA, bonus actions, and game controls are not part of the claim workflow.
Choose legal-document requirements
In the default-language Form tab, review US Tax & Affidavit Documents:
- Automatic for US winners over $2,000 is the default product setting. Country is required and cannot be removed because it determines whether the document package applies.
- Always require requests the package for every applicable claim.
- Never require disables this automatic package.
These are Revup's collection settings, not a statement of your legal or tax obligations. Confirm the documents your promotion needs with your own adviser. Do not collect tax documents or signatures merely because a control exists.
Set notifications and languages
Under Settings β Notifications, review the administrator submission-email option, which is on by default. This is separate from sending the winner their claim link.
Manage available languages in the main promotion builder, then review each claim-language version. The claim builder can edit its own translated labels and submitted messages, but does not add or remove promotion languages.
Preview without creating a real claim
Use the preview's Form, Form with sample legal documents, and Submitted states. A tokenless preview can validate fields and show the submitted screen without storing a real claim. Reloading returns to the Form state.
Then test a dedicated staging winner through notification, the private claim link, submission, response review, and fulfillment. Claim links and answers can grant access to sensitive personal information; share them only with the intended winner and authorized administrators.
